Doctor Khumalo was born to Eliakim and Mable Khumalo on June 26, 1967 in Soweto, South Africa. His late father was a pillar and founding member of the Kaizer Chiefs. In the 1960s and early 1970s, his father was a skilled midfielder who played for Kaizer Chiefs and Moroka Swallows.

Khumalo began playing football professionally in 1984 with Swallows Reserves. Later, he joined the Kaizer Chiefs junior team and played for the club.

A year later, he was elevated to the senior team by coach Ted Dumitru. He became a star player for his squad thanks to his unrivalled show of football talent. He did not play for any other football team, but he did travel for a few brief stints to play abroad. Doctor was named the 1992 South African Footballer of the Year thanks to his accomplishments and football prowess. He participated in 397 league and cup games for Kaizer Chiefs, scoring 90 goals.
